Registration Form

We require a Parent or Guardian’s Signed Consent on the first visit for treating minors under 21.

If you are unable to accompany your child / ward, you may fill and sign the digital registration form sent via email or to your mobile phone on request.

Medisave Form For Claimable Surgical Procedures

Certain procedures such as Wisdom Tooth Surgery are Medisave-claimable.

You may use your MediSave to pay for yourself, your Spouse, Children, Parents or Grandparents or Siblings’ medical expenses, as long as they are Singapore Citizens or Permanent Residents (SC/PR). Please refer to the CPF website for the prevailing Terms and Conditions.

If you are paying for the patient but unable to accompany them, please print and sign the form in advance and send us a WhatsApp screenshot to check if it is OK.
